MSA Envelopes for Memorial Services


The MSA Envelopes are intended for distribution at memorial services.  They will be sent via FedEx in lots of 25.  Please do not request an envelope for an individual donation.  

In lieu of flowers, please request MSA Memorial Envelopes for tax-deductible donations in memory of a loved one, family or friend who suffered from multiple system atrophy.  MSA Memorial Envelopes can be requested by calling the Support Group Hotline at (866) 737-4999, by completing the request form below, or by downloading a PDF form.  The Memorial Envelopes will be Federal Expressed overnight to you. Acknowledgments will be sent to the family and the gift and donation givers. 

You can also set up a dedicated online Multiple System Atrophy Memorial Fund page in honor of your loved one at First Giving.  The MSA Support Group is a registered member of First Giving, which makes it very easy for individuals to set up secure Internet based MSA Support Group fundraisers.

Why Donate to the SDS/MSA Support Group

The Shy-Drager Syndrome Support Group has a continuing goal to increase both the number of donors and also the total dollar amount collected each year. The money raised will be used to fulfill our mission of providing support and education to those affected by SDS / MSA. In order to achieve our mission no donation will be to small or to large for a grateful acceptance.


Proceeds will be used to fund Support group initiatives such as education, awareness, advocacy and research. A minimum of 25% and as much as 75% of each donation received will go specifically to the MSA Research Fund.  Click here to learn about the MSA Research Fund.  The remaining money largely goes to education and patient/caregiver support.  The MSA Support Group is a 501(c)3 charity and all donations are tax deductible. Corporations are encouraged to sponsor "The Corporate Circle of Hope." In return for the financial support companies will receive sponsorship recognition, including logos and links to their websites.
